It appears as though your installation is corrupt. As a quick workaround you can try the instructions in the following link. Please note the link below is labeled for Engage but applies for Quizmaker as well.
Please ignore the the instructions regarding Microsoft .NET Framework 2.0.
If the above link does not help, please reinstall the Articulate Studio. Make sure you restart your system after uninstalling and again after installing.

Does that issue occur on all images or just that one image?
All images. Try insert *jpeg, *png files

Are you working off of a Mac via Parallels or a virtual machine? Also, are your image files located on a network or USB drive?

the problem was in powerpoint trial version. buy ppt and problem was fade away.
