Is it possible to set up a multiple response type question where learner earns one point for each correct choice, and negative one point for each incorrect choice? for example:
1.Which History of Present Illness (HPI)
components are documented?

Location (+1)
_X_
Quality (+1)
_X_
Modifying factors (+1)
_X_
Associated signs/symptoms (+1)
_X_
Duration (+1)
___ Context (-1)
_X_
Severity (+1)
_X_ Timing(+1)

Storyline doesn't allow for scoring by choice within multiple response questions. Although you could use a similar method as the link Leslie provided and set it up with variables as in this example Dipti shared.

In Quizmaker '13, you need to look at question scoring by 'By Choice', which is explained in this tutorial.
This is also able to be accomplished in Storyline as well, and you can check out this article.
