Can you check to see if you have installed the latest update for Quizmaker ’09? And then republish your quiz.
If that doesn't help, check to make sure you have removed any spaces and/or special characters from the file path to the quiz.html file. Please let me know if that helps....

yes i've the latest version of Quiz Maker
And no spaces on the file path to the Index_lms.html
