I am attempting to create a survey, I was able to enter the group title but when I a selected to add the first survey question, and them selected the "pick one" option, I was unable to type in the window to enter the question. I also was unable to type the choices that the user was to select from. It's like these fields are locked. I can't type or paste text in these fields. Can someone tell me what the problem is.

Sounds like something is going on for sure. Have you closed QM and tried to re-start, or even re-booted your machine to see if it helps?
Have you been able to use it successfully before or was this a first attempt? What version of Studio are you using?
