2nd attempt = less points (again)

I'm running into an issue where the learner is credited fewer points if they have entered the correct answer on the second attempt. I don't want it to function this way- I'd like them to be awarded full points, as this is more of a guided learning activity than an assessment.
So for instance, I have four questions. If I get one wrong on first attempt, but correct on second attempt, my score is displayed at 80% instead of 100%. As a default, 80% is our pass rate, unless it's a compliance or HR course.
Has anyone solved this? I found one answer on the boards from 3 years ago that stated he solved it through branching from the form view, but I can't see any way that would help. Any ideas?

Unfortunately, I can't upload publicly for privacy reasons- I have uploaded as a case though - 01772715. Thanks for looking into it. It's really frustrating.
To update the public discussion: I noticed that there were 5 questions tracked by the results slide, and it seemed likely that getting the 5th question incorrect was impacting the overall score.
Please let me know if testing that idea doesn't match what you're seeing!
