here is a sample custom likert survey I did in SL1 for someone else. It has the actual responses logged on a 'final slide' but in your case you could add a number variable to each response for each question and then calculate a total score if that is what you are after.
here is a sample file in SL1. It has two survey questions on the same slide - the participant selects a response for each statement which triggers a True / False variable. They click submit and the next slide has a number variable that is adding up the 1,2,3,4,5 for each button click for each statement. Hope this helps you
What is the current status with being able to report on Likert variables/responses?
I seem to remember that AO was the only (?) LMS that was really happy reporting on them. I tried to use them once and while it was all fine for the "input" side, my LMS could not report on them so we had to find another way.
I don't see a KB article for Storyline 2 like there was for Storyline 1 in regards to the issues with Likert questions. I also checked with the reports filed with our QA team and don't see anything there to indicate it's not reporting to LMSs. And for Storyline 1, there was a fix included in Update 6 for Likert issues and not reporting to the LMS.

The likert I'm trying to create has 5 options
strongly disagree = 1, Disagree = 2, Neutral = 3, Agree = 4, Strongly Agree = 5
The next slide would total their responses
I have no idea on how to set these variables
