A paper questionnaire

I have a pdf of a questionnaire with a few pages. I plan to show how to fill this out via call-outs. Just working out the process to do this. Would I print out the form and fill it out by hand (that's how it will be filled out) and then find a way to upload it? 

Would I take a screenshot of the pdf pages to show in a scroll bar to show the questionnaire initially blank?

Hi Susan

If I were doing this, I'd fill in the form on a tablet (so you get the handwriting), but take a screenshot after completing each field.

I'd then create a short animation/movie in something like Camtasia, which will allow you to add in the callouts.

Alternatively, you could create a building animation using triggers and slide layers.  If I were going down that route I'd put a "1..2..3..4..5..........." or "<....>" at the bottom of the page that the user could click on to take them through each stage.  If you play around with the animation on the slide layer you can make it look quite smooth.
