I have figured out that if I select "Never" in the "Include in Shuffle" collomn in my question bank, it becomes impossible to preview. It only loads the timer running when I want to preview, and nothing happens.
But if I leave this setting in "Random" I have no problems with preview. However in order to trigger this problem I must change the setting to "never" for all of my questions. Leaving only one "Never" and the rest "Random" does not trigger this problem.
Please correct this. Im using latest version of Story line

The image you are sharing is for your question bank and setting the questions to never display, not related to the answers being shuffled.
So, if you are setting the questions to never display, I would expect you to get a blank preview.
If you were hoping to set the answer shuffle option, you can do that per question.
