Any idea about How to make a Form in Articulate?

I am planning to create a template in Articulate Storyline that will:
1. Capture data from 6-12 free form text fields. Four text fields are located on three different pages.
2. Place the data in the last slide of the storyboard template. Allow the data to be edited if the user wants to.
3. Take the data from the 'last slide' and allow the user to click a button and have the data in a Google, Yahoo AND OR Outlook Calendar invite.
