Hi Richard! I apologize, but that screen shot does not look familiar off-hand. What is the URL/website/navigation method to get to where you are seeing this?
Are you running wordpress? There may be some code in the source file or you could have accidentally added a widget to the sidebar and this could be causing some issues.
Unfortunately, I haven't solved the problem. It still appears on my contact form. I had designed this page to help me land a new job and since that has happened, my attention has wandered. Please come back and post here if YOU should find a solution!

The page WAS built in WordPress, and I used a template provided by a "Hero". But... that sentence wasn't there at first. It just showed up lately.
The only Widget I've added is the Video Editor, and I used the ADD CONTACT FORM button to build it.
