By default, Storyline does not support evaluating more than one data entry field on the same slide. However, you may be able to evaluate multiple fields on the same slide by disguising a Freeform Pick Onequestion as demonstrated in this video tutorial:
And if you want to evaluate multiple fields on the same slide with multiple possible correct answers, take a look at this video tutorial:
