Conditional / Branching logic with Dates

Hello, 

Im designing a set of instructions that is time dependent 

See the attachment for wireframe depiction. 

Example:

A person has set appointment on September 7. They need to follow a set of timed instructions before their appointment

I need them to input their date September 7th and this will generate a set of instructions for them that appear on a separate slide

This creates a slide with complete timeline schedule of instructions leading up to the appointment with correct dates:

The goal is for client to have a clear picture of when there supposed to do certain things

TAB ONE - ONE WEEK PRIOR TO APPOINTMENT: Logic would be (appointment -7 days) - Please change your diet to the following. 

TAB TWO - 3 DAYS PRIOR TO APPOINTMENT: Logic (appointment day - 3 days would appear) Please Hold the following Medications

TAB THREE - 1 DAY PRIOR TO APPOINTMENT: Logic (appiontment day - 1 days would appear) Please change your diet and review video. 

TAB FOUR  - DAY OF APPOINTMENT etc...
