Course Completion Sign off Sheet

Hello,
I know that this subject has been raised before but I have not found anything that helps me in what I am looking for.
I am starting to develop a lot of course using storyline 360. The course is usually accessed by the end user via our LMS. I have found a way of including a course sign off sheet at the very end but I am looking for something a bit more complex.
What I would like is for the end user to type there details at the end of the course and then have that information populate one of our companies sign off sheets.
Our LMS does not have provide a very good sign off so hoping I can do something in Storyline.
As I mentioned I already have it so the end user can print the the sign off but now I am looking to have their information included automatically.
Any ideas?

Hi Daniel,
I don't have an answer for you, but I do have a question: how did you incorporate a course sign-off sheet? My client wants review of policies to end with the learner typing in their name (as opposed to clicking OK) to indicate they understand and accept a policy and I don't see how to do this.
