Designing a Survey page and receiving the information direct from Storyline

Hi Everyone,

I would like to design a "Survey" slide at the end of my course and receive users feedback through Storyline.

For example:
User fill the Survey and press "Submit",
and information get transferred to me as an email or goes to a Database

Any help is appreciated,
