You can build a survey directly into your Storyline course by using form-based questions. Keep in mind that you'll need to add a Results Slide at the end of the survey to send reporting and tracking information to your LMS, including learner responses.

What that in mind, you can always remove the results information from the results slide and design it to be the last slide of your course as a "Thank You" or "Congratulations, You've completed...", just as long as you keep the Submit Results trigger when the timeline starts.

One more question. On the feedback slide, I have a likart scale and an additional comments section. How do I ensure the comments section (I created it as an input text only), can be captured within the results/survey?
I don't think you can put both a Likert-style question and a text-entry survey question on the same slide and get them both reported.
Perhaps consider separating them out into different slides? Hopefully an instructional designer in the community can offer their ideas on how to go about this!

I do have a Results Slide, but I don't want one for Learners, but for my own feedback (which was originally attached as a screenshot for reference.

Trying to follow the instructions above. Can you clarify how I can set up this survey results to send the response to an external email address?
Another option may be to create a final survey slide that would embed a Survey Monkey survey for the learner to voluntarily fill out.
Any suggestions would be great.
