I found storyline template with smiley faces and a sliding bar. I want to use that template as a L1 feedback form where learners can rate and tell us how they feel about a piece of e-learning. We would like to upload the file onto our LMS and be able to pull a report with the feedback when we need to.
For the LMS to be able to get this information, there needs to be a results page and for a results page, there needs to be a quiz or survey.
You can pass the value of the slider to a 'How Many' survey slide if you would like to record the learner's response in an LMS.
Passing the value of the slider is essentially assigning an answer to the survey slide before it is submitted and the response sent to an LMS. Here's is what the LMS will record if this method is used:
