When you add a new quiz slide , you'll have a few different quiz types and options to choose from. If you've set up a particular Question layout within your slide masters, you can also apply that to a quiz slide, by right clicking on the slide within the navigation pane or anywhere in the slide workspace, and scroll to Layout, and pick the desired layout.

Chuck, can you use the "Convert to Freeform" option on the Insert > Slide tab?
