Inserting a survey and a quiz in the same module
Feb 27, 2013
Hello Articulate Community,
We have recently been discussing that we would like add a simple survey to each Storyline module, including modules that have quizzes. The current struggle is that Storyline only uploads 1 results slide into our LMS (LearnShare). I have tried creating a quiz with a results slide, and then inserting the survey. The problem that I am running into is that it locks the survey questions to where they cannot be answered if they are located after the results slide.
Is there a way to design courses with a survey after the quiz results?
If not, I have found that even putting the survey before the results, it erases all survey answers if the learner needs to retake the quiz (which many of our courses allow the learner to do). I have set the slides to "Resume to Saved State" but the answers are still erased. We want to make sure that the learner does not have to fill out the survey again, even if they retake the quiz.
Has anyone successfully inserted a quiz and a survey in the same course?
Any help would be appreciated. Thank you!

Howdy Joe,
If no one has said it yet, welcome to the Heroes forum! I've put together a Screenr screencast going over a way you could accomplish what you are looking for. Take a look at it below:
Basically, you will need to take advantage of Storyline's ability to create multiple result slides, including one that tracks them all. This way, you can get around your LMS's (all LMSs) limitation of only being able to track one result slide.

Thank you so much! This was exactly what we were looking for!
We were able to see both the quiz results and survey answers using this method in our LMS. All of us really appreciate you helping out our team.
Howdy Joe,
I'm glad it worked out! You are most welcome.

Thank you Adrian. This was a big help. Do you happen to know if there's a way to separate the quiz and survey results when looking at reports in Articulate online?
Hi Thomas,
Thanks! At this time, there isn't currently a way to do so. You could export the reports to .csv and then manually separate/sort them that way, though.

Thanks again Adrian. A visit to the feature improvement suggestion page is in my near future.
Since this was a long time ago, just checking to see if any of the more recent Storyline updates addressed this in a better way, or if any developer has found a cleaner workaround, than having a quiz and a survey resulting on a single slide. Just from the perspective of IDT, it's really critical in 2022 that we're able to include both an L1 evaluation (survey) and L2 evaluation (quiz) in a way that's intuitive and engaging for learners.
