Hi Pauline and welcome to E-Learning Heroes :)
That yellow pop-up is to display the actual scale label for the number so that users can choose the correct response.
It looks like your labels may have been removed, which is why it's showing up empty.
Your set-up looks as if it's already been customized, but I'm sure someone in the community can help you with your design if you'd be willing to share this slide in a .story file.
