I am looking to create a question where the learner picks DO or DO NOT questions with several conditional statements in table form. To successfully answer the question they would click on the "x" number of correct hotspots. I cant find a template or graded question or survey which could achieve this ? Is there a way to do this? Has anyone done this type question before. I am not looking to grade it , just to give feedback if correct.

you could set this up as a Pick One freeform question. Use offstage buttons correct and incorrect to control the triggers and no hotspots needed.

You could use a Freeform Pick-Many question for that. See here for info.
