Multiple issues: Drag & drop, Restart Course and final exam layouts

I've recently run into the following issues, and wonder if anyone else is also experiencing these. I am using Storyline 360, and am on the latest version, but have experienced these in earlier versions as well:

• Drag & Drop correct and incorrect states not working - when I drag and drop the item, and then click submit, the state does not change at all. I even added triggers to change to drop correct or incorrect based on where the item is dropped (this should not be necessary). Still didn't work. I have to add custom states and triggers to change the state of the item.
• Restart course not resetting Restricted menu - in our courses, the menu is set to Restricted, and the user must restart the course if they do not pass the final exam on the 2nd try. When they click the Restart Course button, it is not resetting and locking the menu. All slides have the checkmark, showing they are all available. The user should have to go back through the course from the start.
• Question bank slides randomly selecting a different layout after saving and reopening file, or during publish - I created a new final exam question layout for an existing course. After publishing and reviewing the course, several of the exam questions were showing a totally different layout - and not the previous layout either. Just some random master slide. After correcting the layout and republishing, some slides still are showing a different layout.
