Problem with Quiz scoring on LMS

We have developed a randomised quiz. In this we are pulling and displaying 8 questions from 4 different pools. Each pool has different number of questions and in total there are 21 questions in total in all pools.We also have retake quiz option for the user if he fails the quiz. While retaking the quiz. We have question numbers displayed for each question. E.g. Question 1 of 8, 2 of 8 etc. In our case when we are retaking the quiz when we don't pass, the question numbers were getting disturbed sometimes. It showed 9 of 8 or 6 of 4 etc. 

The issue which was reported above is now resolved after upgrading to the latest Storline Version, however when my client is uploading on their LMS, it is showing 112.5% and even at times when the user has scored 100% it is showing the score as 0% in the LMS. We are publishing to SCORM 1.2.  

What could be the issue?
