Why is it that when creating a freeform pick many question that there is no marker indicating where the picture was clicked on? It would allow the exam taker to "un-select thier suspected wrong answer.
By default, items in a Pick Many question have a Selected state with a slight glow that shows which item was selected. You could edit this state anyway you want to.

It should add a selected state on the object, this can be changed to make it more obvious.
