My LMS (LearnUpon) is reporting the information, but I would like to get it in a more useful format. Is there a way to set it so that each line of the Likert scale reports as its own question?
Alternatively - is there a way to change the reporting from "agree" to "4" (numerical rather than words)?
Thanks for reaching out and sharing what you are seeing in your LMS. It sounds like it's reporting on the Scale Label, so you should be able to adjust these values as needed:

Likert scales in most LMSs do not report meaningful data, Moodle is one of those LMSs.
