Text and Checkbox Triggers

Hey everyone, 

I am trying to set up a workaround for an e-signature within storyline. Essentially a user has to check a box and then type three values before the exit course field would appear (see attachment). This is to trick my LMS to ensuring this completed before allowing a person to take my CBT. The problem is that I cannot set it up such that all fields must be true before the Submit and Exit button appears. I tried creating multiple triggers and even layers. However, the it won't let me group text entries. Any help would be appreciated.
