Look in the final package's story_content folder to see if it is copying it. Sometimes if you move the location of a media or attachment file while working on the project, it doesn't copy it to the final product.
What actually happens when it's on a server or LMS? An error? Blank box? Endless loading icon?

You might also want to check with your web/LMS admin if the webserver/LMS is configured to support the .mp4 MIME type.
