I would like to create an assessment that requires the learner to review a document and click on the mistakes. They would have to identify all of the mistakes but only get a limited number of attempts. Has anyone else done this or have ideas on how to do it? Thank you.

My first thought is a freeform pick many question type. This is a built-in free-form question type.

You can control the attempts with this question type as well:
