Want to Print Certificate on course completion. The name to be entered by user . certificate will have current date.

I want to print the certificate after course completion. The name of the user to be entered by user. I want to add image of the certificate on which the name and date will get printed. I am attaching the certificate image. Please guide me with javascript.
