So you might find something helpful or a juicy tidbit in one of those articles. I've had a peek at your document and it looks like you're covering a lot in it. Perhaps your cover letter can be a simple resume of the what, why, when, and how of the training requesition form. What it is, why it needs to be filled out and what's the goal or purpose of having the information. You might also consider adding some instructions or directions for the user on how to fill it out and where to send it to/submit it when it's done.
