Engaging Feedback Forms

I am about to start working on a project based on feedback elements of e-learning and how these can effectively gather useful and constructive feedback based on asking the right questions and also to make these feedback 'sheets' more aesthetically pleasing!  

I am looking for ideas on better ways to ask 'How could the e-learning have been improved' ?.... 

Any help would be appreciated! :-D

I'd suggest taking some time to figure out what specifically you (or the project's sponsors or stakeholders) want to know. Then break that big idea down into a few smaller, more targeted questions.

For instance, you could ask about the length of the course, the difficulty level, whether the learners found the content relevant, etc. instead of asking about "the e-learning" as a whole.

Google searching "survey writing best practices" should pull up a number of useful resources.
