I need to ask a survey question that requires the learner to answer with month, date, and year. Can I create a survey question that requires them to answer the month, date, and year on the same slide preferably using a drop-down format.
Have you thought about having them type in the date instead of having drop-down menus? You could do that quite easily with a short answer survey question.

Yes, That is what I did but my client prefers having the learner select the month, day and year instead of typing the response.

Another option would be to create a custom set of drop-downs using layers. Here's a video that explains how to do that.
