But I am still unable to add items. Neither "first" nor "second" edit button appears. I am using Chrome. I have checked in Firefox as well - to no avail.

The Authoring buttons were recently changed. The link you included refers to the old buttons. The new buttons are shown in your screenshot in the top left corner.
Click the Content button.
Then click on the image to add markers.
Click Edit below the image to change the image.

Would be great to have this instruction in the shaded box, e.g.
Click the image to add the marker. Then click Edit button at the bottom of the image to add annotations.
