Issue with Quiz Review

I've been facing a challenge for the last week and I'm seeking assistance from anyone who can understand the issue. I have a quiz consisting of 5 single -choice questions, each question pulled from a different question bank. While the quiz itself functions correctly, I've noticed an unexpected behavior when reviewing the quiz by clicking the review button. Some slides display multiple selections, even though I only made a single choice per question. Please refer to the attached file for more details. I would greatly appreciate any help or insights.
