OJT Tracking Worksheet/Form that is SCORM compliant

We require a worksheet to monitor the progress of an on-the-job training (OJT) exercise. The worksheet will be launched and completed by the proctor from our LMS. For each step of the exercise, the proctor can select either "Yes" or "No." At the end of the worksheet, the proctor can select passed or failed. A text entry field will be available for comments. It is essential for the worksheet to be SCORM-compliant so that the data can be tied to the student's account. (Our LMS includes an OJT feature that allows proctors to initiate and complete packages on behalf of students.) We are uncertain about the best approach for creating the worksheet. One possibility is to begin with a Google form, although integrating the SCO aspect presents a challenge. While we are familiar with Storyline and Rise, the primary hurdle seems to lie in developing a form-like interface. We would greatly appreciate any shared comments or experiences relevant to this challenge.
