Stop Callout "Triangle" from deforming?

I totally understand your dilemma. If you want the exact precision you are showing here, then you may want to consider building it with multiple objects instead of the call out option.
If you choose the call out option from the Shapes menu, you are really limited to 2 base locations on the side of the rectangle. Any other position of the pointer other than centered over one of those base locations and the triangle warps.
Choosing a call out from the Captions menus gives you more flexibility, but still probably not what you are looking for.
