Suppose user is accessing the rise files with take again feature but he got failed.
Then he clicked on take again button to retake the course but completion status is not clearing from failed to in-progress or unknown. every time it is coming failed only.

Would you mind taking a screenshot of your publishing settings so we can see how you setup completion tracking for your course?

Additionally, you'll want to check out which reporting option you're using for your course, as each option affects LMS reporting differently.
If you find that one option is not reporting properly for you, try using a different option to compare results.
