Video overplaying

I have created a quiz question with a video mp4 embedded in the question. When the video is played, it plays two separate videos at the same time. I deleted it, now it over-plays the same video over itself, so you hear the same audio playing twice at the same time. I created a new question and uploaded the video, now it plays three times! Has anyone encountered this before? Help!
