Audio and Video not playing in Published LMS (aicc) version

Hello. Can anyone please help with an inconsistent issue with audio and video files?
I have created a package using MP4 video files and mp3 audio files and this has been published in WEB format and located on the intranet. There seem to be some inconsistencies in the ability of some people to view the video files - which I'm hoping may have something to do with MIME settings and our IT department are looking at this at present.
However, I also need to embed the module in the LMS for other learners who need to have their competency recorded, and I (as the developer) cannot see the video files, but more worrying is that the audio (mp3) files do not play EXCEPT in the quiz sections. All the audio is the same format.
Would appreciate any ideas anyone could offer.

As far as the audio issue, is it only missing in the published version? Is it present when you play locally?

The audio is working locally, also over our Intranet. We have had some inconsistent issues with the video files in other locations, but when published to the lms is the only time the audio isn't working (but even that is inconsistent because it works within the quiz sections).
