Non-Anonymous Surveys

I need to create a survey but I have to know who took the survey and exactly what their answers are? Everytime I do a survey it makes it anonymous and I can't tell who answered. Obviously for most surveys you want them anonymous but is there just a way to use the survey and see how the people answered. I.E. what office supplies do you need. Do you need a new computer, do you need a new chair. If they say they need a new chair I need to know who needs a new chair etc.

Use a 'short answer' survey question type at the start and have them type in their name
