Question not showing during Post-Quiz Review

I have a standard multiple choice quiz for which I'm using a custom template. I created the template starting with the "Clean" theme, modified it, and saved it under a new name as a theme as well as a template.
I am using the template for new quizzes, but am having trouble with the Review Feedback layer. The Review Feedback slide layout is applied, but the question does not appear on the slide, just the rectangle box with the feedback text that I entered in the Post-Quiz Review in Form View.
In troubleshooting this, I created a new quiz using the default template ("Clean") and the Review Feedback layer shows up fine, so I did a side-by-side comparison of the settings and can't see any difference between the one that displays correctly and the one that doesn't.
Why won't my question show up on the Review Feedback layer?

Without seeing the files, it's hard to say what could have happened - but you'll want to confirm that you're working on project files as described here and following along with these steps in terms of customizing your slide masters and feedback layers.
