Quiz answers out of order

Hello

I have 3 quiz attempts from a user. The results for the first attempt show Questions 1 and 2 but they are actually Questions 4 and 5 in my quiz. Attempt #2 has an answer for the correct #1, and then her final attempt has all of the questions in the correct order. I did not randomize the quiz questions and the quiz is set up for 1 attempt and no skipping, so do you know how she was able to answer questions out of order on her first attempt?
