Unable to acquire LMS API

Hi everyone,
I am trying to publish an Articulate Quizmaker file to our LMS Success Factors. I need to load the published file into an FTP site and then connect the FTP URL to the LMS to launch it. When I test the index_lms, I get an error (see attached). It seems that the indext is not connecting the blank.html file in the LMS output folder. If I launch from the quiz.html, it plays fine but does not record scores in the LMS. Also this files was an older version of Quizmaker. Do I have a corrupt file? do I need to rebuild? any ideas on how to get this course to launch and run properly?

I'm not familiar with these particular errors, but this troubleshooting document covers many LMS issues that can crop up.
I would advise testing your content in SCORM Cloud to help you determine if it is an issue with the content or with your LMS.
