Articulate Storyline course on Wordpress LMS

Hi all,

We've developed a course that needs to gather a users name, email, address and phone number and at the end of the course, export this information out and email it to a particular email address.

The way I have set this up at the moment is by having text entry boxes which are then used to populate short answer questions which turn into a result slide (in the hope that this result slide is tracked by the LMS and the answers accessed by the tin canny reporting plugin).

This set up does not appear to be working. When the course is uploaded to learn dash and completed through the Wordpress page it's placed on, tin canny does not even track that the course is completed and I can't find any reference to it gathering any results from the course. 

Am I going about this all wrong? What is the best process to do what we require in the first paragraph on a Wordpress site?

Has anyone done similar and can provide any insight?
