I have an assessment at the end of my course. When I select the "Review" button, it is showing a RED INCORRECT bar at the bottom, even though the question is correct, and it is logging as correct in the results. Has this happened to anyone else?
Also, the Assessment won't unclock in the menu after it's complete?
Without seeing the .story file, my initial thought is that your questions are set to "Reset to initial state", which is why you also don't see the answer selected.
Changing it to "Resume saved state" should solve the issue.

I am attaching just the Assessment. I had to strip out some info as it's confidential.
You will notice after reviewing the scene, the Assessment is still locked.

We have a bug reported where returning to a question bank via the menu is not working in Restricted navigation.
I've connected this discussion to the bug report, and I'll be sure to come back to update you when I have some news.
If it works for your design, the workaround is to use question slides instead of a draw from the question bank.
