Building a scored checklist in Storyline to add to our LMS

Hi there. I want to create a scored checklist to document whether someone completed a task and also have a section for comments. These tasks will be grouped by topic. The instructor will observe the learner and mark yes or no for each task, and provide comments if necessary. I would like to get a score at the end based on the number of YES responses (a %). I want to add this "course" to our LMS (Scorm) so the score will appear on the user's transcript. I have a few questions about setting this up:

1. Is this even possible?

2. Do you have any ideas on the best way to structure this - or possible examples?

3. I started with quiz slides, but it looks like I can only have one "question" per slide. It is possible to have all tasks within each topic on one slide (5 tasks per slide x 5 slides), with space for instructor comments, or does that mess up the scoring?

The built-in scoring function needs to be done on a slide-by-slide basis. And, to send that info to an LMS, you need to have a Results slide.

If you use built-in Survey questions, there needs to be a Survey Results slide.

In both cases, you can redesign the Results any way you want.
