Communicating Survey Results to LMS or via Email

My organization is trying to store opt-in information for new employees. Essentially, we want to present them with a question on whether they wish to receive a vaccine. This will be delivered in our LMS as a scorm file.

We want to be able to see the employee's name and whether they selected "Yes" or "No." I've tried a variety of reporting methods and nothing seems to work. I have seen some solutions that use Javascript to send an email, but I'm not sure if this will work if the employee is using a shared device that they are not logged in to, and may not have a default email client.

I've included my sample. This does include the question as a graded option and a survey question. I tried the graded option in our LMS as a workaround (essentially, if users answered "Yes," they passed, but this score wouldn't be weighted and wouldn't count against their progress--despite my attempts, answering "No" still showed the module as incomplete in our LMS).

Any help or guidance is greatly appreciated!

I took a look at your project file and saw that it was missing a 'Submit Results' trigger in the Result Slide

With this trigger added, I can confirm that your course was able to communicate both 'Completion' data as well as record the response of the learner in an LMS.

I've attached an edited version of your project file for testing. Try uploading and testing it on your LMS, and let me know if you can see both the completion mark and your learner's response in the reporting section!
