Creating a Survey/Quiz for Values

I'm working on a course about Values for church leaders and would like to have an interactive slide or two for them. My thought is that I could list out 20-30 values and they could either pick out their top five or maybe have them rank them.

My other idea is that I could use a Likert Scale slide with 10 values, they could rank them...then based on their rank the next slide would have their top five plus five more. We could do that a few times and they would end up with a slide showing their overall top five.

This is just in my head. I do not know how to do this and would love some help. I'm sure there is a tutorial already out there somewhere...I just cannot seem to locate it. Thanks!
