I inserted a Freeform Pick May slide from the Content Library 360 and made formatting changes. However, when I am unable to select the correct choices (A, C, D,E) as set up in the Form View.
I can select choices C,D,E but if I try to select A, choice C is deselected and vice versa. I've seen some other threads reference Button Sets, but I don't see that issue in the file, but I may be missing something.

I noticed that both parts of the A and C groups were in button sets. So you need to set all those objects to "(none)".
I'm attaching my edits because I also created an object that doesn't need to be grouped. Instead, it adds the small circle directly to the states of the main object.
This avoids the need for a group--which is good, because grouping objects can interfere with how they perform in interactions.
Thank you for finding the problem. I had a difficult time getting the Button Set option to display for A Oval and C Oval. But I eventually figured it out. Thanks again!
When you are in the edit-states mode, you can insert or paste additional objects into any state. You have to be in edit-states mode to edit those objects.
First, select the state you want to edit.
Then select the object on the slide and edit it just like you would if it were a stand-alone object.
Note: You can revise the text for the main object in the Normal state without being in edit-states mode. And, typically, that will change the same piece of text in the other states (unless they were edited to be different from Normal). However, for the "extra" objects, you will have to go into edit-states mode and revise each extra object in each state. That's because they aren't connected in the same way as the main object.
So, for example, if you want to reuse the sample object I created, you could edit the Normal text in the big circle, and that revision should appear in the other states. But for the "extra" smaller circle, you'll need to edit that text in each state.

Thank you for the edits. That is a good tip. But how would I do I edit the text in the smaller circle? or recreate this object?
