Having a final grade based on many quizes

Hey all,

I am trying to recreate the following quiz weighting & grade percentage . Any insight would be greatly appreciated!

Method of Evaluation:

1. Quizzes  40% -The score from each quiz is combined, and averaged to calculate the grade.  There is one (1) attempt for each quiz. 

2. Final exam 60% - There is a maximum of one (1) attempt for this theory exam.

Final Grade = _________/100%

Grade Percentages:

Grades awarded are based on the following distribution:

A+        95 – 100%
