HTML code appearing in data entry fields in published output viewing from Safari 9

Hi! I have a course that includes several data entry fields, with variables associated with each. When the course is published and viewed on a Mac using Safari (v9.0.3 to be exact), html code is visible in all of the text fields. These text fields should be blank, we should not see any code.

I am attaching a screen grab here. I have not seen this issue in earlier versions of Safari run on the Mac, or Chrome on the Mac. Nor have I seen this happen on any PC browsers (IE, Chrome, Firefox)

Any guidance would be greatly appreciated!

Hi, Laurie -- Many thanks for sharing your question and for the screenshot! Please ensure that you are testing your published output in the intended environment, and you may want to check out these troubleshooting tips for unusual behavior, as well.

And if issues persist, you are welcome to share your file so that we are able to run some testing for you. You may either upload here in the forum post, or if privacy is a concern, here is a link to offline review.
